L'investigatore assicurativo John Trent, viene incaricato da un'importante Casa Editrice, la Arcane, di ritrovare lo scrittore Sutter Cane, nuovo fenomeno di massa nel mondo della letteratura horror (una sorta di Stephen King). Il suo ultimo Best Seller è infatti appena uscito e sta già facendo accorrere nelle librerie orde di fans in delirio, ma di Cane non sembra esserci più traccia, scomparso nel nulla proprio mentre stava completando una sua nuova opera, In the Mouth of Madness, che nelle sue intenzioni sarebbe dovuta essere il suo capolavoro assoluto, promettendo incubi mai letti su carta stampata.

Accompagnato da Linda Styles, segretaria della Casa Editrice, Trent si mette quindi in viaggio verso una sperduta cittadina, Hobb's End, in cui si crede che lo scrittore si sia rifugiato: quel luogo sembra uscito esattamente da un incubo Lovecraftiano, un incubo che perseguiterà John Trent fino al fatidico incontro con Sutter Cane, il quale gli aprirà gli occhi su una verità talmente incredibile da sconvolgere per sempre la sua mente, in un climax che termina con uno dei finali più apocalittici che si siano mai visti nella storia del Cinema del Terrore (che ovviamente non rivelo per nessuna ragione).
