Bennie Tetrocini (Alden Ehrenreich), de 17 años, llega a Buenos Aires para buscar a su hermano desaparecido hace diez años. El joven Bennie y su familia (emigrantes italianos en Argentina) se habían mudado a Nueva York debido a la exitosa carrera del padre, Carlo (Klaus Maria Brandauer), un renombrado director de orquesta sinfónica. Cuando Bennie por fin encuentra a su hermano mayor, el brillante poeta Tetro (Vincent Gallo), se da cuenta de que es todo lo contrario a lo que él esperaba. En lugar de ser el hermano maravilloso que fue el ídolo de su infancia, Tetro es ahora un hombre distante y desilusionado que cambió su nombre, no quiere saber nada de su familia y ya no escribe. Durante el tiempo que Bennie pasa con su hermano y la novia de éste, Miranda (Maribel Verdú), los dos vuelven a vivir las experiencias tormentosas de su pasado y se revela un secreto terrible.
