Fred Madison est un saxophoniste plutôt aisé de Los Angeles. Cependant, il soupçonne sa femme Renée de le tromper. Très vite, après le début de ses soupçons, il reçoit des vidéos. Filmées par un inconnu, elles montrent l'appartement où il vit avec Renée, vu de l'extérieur puis de l'intérieur. Après la visite peu rassurante d'une équipe de policiers, une nouvelle cassette vidéo montre Fred à côté du corps de sa femme assassinée. Il est alors condamné à mort pour ce meurtre mais un homme mystérieux l'arrache à ce destin par un moyen inconnu. Fred Madison se retrouve alors dans la peau d'un autre homme mais, comme dans un rêve, les éléments de son passé vont peu à peu réapparaître, sous une forme différente.
