Den gamla fängelseön Alcatraz (The Rock) ockuperas av elitsoldater ledda av överste Francis X Hummel (ED HARRIS). Med turister som gisslan riktar de raketer laddade med biologiska stridsmedel mot San Francisco.
Mannen som kan rädda stadens invånare från en säker död är John Patrick Mason, den ende fånge som rymt från Alcatraz och överlevt, spelad av legendariske Sean Connery. Han får hjälp av Stanley Goodspeed, en begåvad men virrig FBI-agent, spelad av Nicolas Cage.
