April Meddows is een medisch rechercheur in dienst van de Amerikaanse Communicable Disease Unit CDC. Haar huidige onderzoek omvat het raadsel achter de uitbraak van een mysterieus virus. Dit virus, eerst beschouwd als E-bola, besmet de geïnfecteerde met een uitzonderlijke razernij en uiteindelijk een weerzinwekkende drang voor mutilaties en moordzucht. Het raadsel achter de oorsprong van deze ziekte wordt echter nog groter wanneer er geen enkel spoor van besmetting wordt aangetroffen in de lichamen van de besmette personen. De enige aanwijzing leidt naar een oud schilderij, die de poort naar een andere wereld schijnt te bevatten...
