Da kommt es Niko gerade recht, dass sein Cousin Roman in derselben Stadt lebt. Als Niko in Liberty City ankommt ist nichts von dem so, wie ihm das von seinem Cousin versprochen wurde. Das Einzige, was Roman reichlich hat, sind Schulden bei der organisierten Kriminalität. Und so wird Niko in einen Strudel aus Intrigen, Gewalt und Drogen gezerrt.
